The Prior Record Worksheet has 40 offense categories, with spaces to enter both the number of adjudications and the number of convictions for each offense category.

If a Prior Record Worksheet(s) for an Offender with the same social security number already exists in the system, the worksheet(s) can be viewed and copied for use in the current Judicial Proceeding.

At the bottom of the page, the Prior Record Score Calculation Results display automatically. Possible Prior Record Scores for both lapsing and non-lapsing conditions are calculated, since the offender’s age at the time of the Offense(s) may not yet be known.
